Overview

Calculated demographic data show a rapid increase in chronic diseases and polymorbidity due to the ageing population. In future, healthcare services will no longer be able to be covered by social security providers in their current form. Instead of the structures that are primarily geared towards acute health problems today, new care concepts geared towards chronic diseases and long-term care needs must be developed and introduced across the board. The central task here is to support the translation of knowledge (theory) into action (practice). The initiation and implementation of modern approaches often prove to be a very complex and multidimensional process in practice due to various barriers. The challenge will be to narrow the gap between knowledge and action so that nursing practice and nursing science are no longer presented as two different worlds. Opportunities to support change are being sought through comparisons with pioneering countries at the international level with regard to the job profile of ANPs (Advanced Nursing Practitioners).
