OverviewIf you take care of someone who is elderly, disabled, or has health issues, You need to be documenting every day as to what transpires. You can show that Dads blood pressure has steadily increased over the last 6 months and today's high reading is not an isolated reading. When problems occur it is a lot easier to grab the BOOK rather than hunt for all these things and get to ER only to find that you forgot the medication list.
