Overview

Published in 1999, the first edition of The Psychotherapy Documentation Primer became a standard for professionals and students who needed a quick and easy, yet comprehensive, guide to writing a wide range of mental health documents that meet third-party requirements. Intake, assessment, treatment planning, progress notes, and more were all covered in a straightforward, easy-to-understand format. This new edition improves on its predecessor in two important ways. First, it has been fully updated and revised to include many changes that have occurred in mental health practice, including: changes to DSM codes and definitions; new managed care guidelines; radically changed accrediting requirements from JCAHO and CARF; and the new Federal HIPAA regulations. Second, the book has been revised to be more relevant and useful for the classroom with the inclusion of more examples; more and better graphics; study questions at the end of each chapter; and an expanded references section.

DONALD E. WIGER, PHD, a psychologist, runs a large mental health clinic in St. Paul, Minnesota, and is an adjunct professor at Argosy University, The Minnesota School of Professional Psychology. He is the author of numerous books, including The Clinical Documentation Sourcebook, Third Edition, also from Wiley.
