OverviewAdvances in neurosurgery have illuminated the importance of the role of the venous system. In the past the main focus of neurosurgery has been directed to arteries such as those feeding tumours, although it has become clear that some lesions that had been considered contusional haemorrhage caused by surgical manipulation were in fact haemorrhagic infarcations resulting from impaired venous circulation. In addition, the concept of venous hypertension has emerged as a mechanism of neurological impairment in patients with arteriovenous malformations in the brain or spinal cord. Attempts have also been made in neuroradiological diagnosis with the introduction of magnetic resonance imaging. For example, the exact location of tumours and the degree of invasion of sinuses can be determined prior to surgery. Thus, new operative techniques are being developed that benefit from such detailed information.
