As a practitioner and student of the tools of analysis, this book is an indispensable resource in finding the right tool for your need. A true What's What on the best approach for identifying gaps, refining processes or identifying the latent hazards that tend to emerge as injury, loss and/or damage. I know the term Root Cause isn't currently voguish but don't be fooled by the modern hype. The methods and tools discussed come from more than 50 years of experience at analyzing, prioritizing and removing the process gaps, oversights and breakdowns that undermine reliability. Just like any healthy tree or plan has an extensive root structure, the methods explained help you logically and systematically identify those roots that are undermining the health of your organization, allowing you to prioritize which of those roots will deliver the greatest return on your investment. I've personally used the methods described in Root Cause Analysis: Improving Performance for Bottom Line Results in investigating injury provoking equipment failures within heavy industrial systems and processes. The focus on how can and logical hypothesis testing recommended were appropriate and eye-opening in identifying the true factors of influence that resulted in clear and actionable corrective actions.
