Overview

Bruno Speck was a pioneer in the treatment of severe aplastic anemia (SAA). This special issue in his honor comprises individual contributions by clinical and experimental hematologists who had known Bruno Speck as a colleague and friend. They describe the history of SAA from a lethal to a potentially curable disease, the development of antilymphocyte globulin (ALG), and its transfer ‘from the rabbit to the patient’ - Bruno Speck’s major contribution to the treatment of SAA. An overview of the state of the art in diagnosis and treatment of SAA is given, with special emphasis on the chances of cure. New results on the intricate relationship between SAA and the clonal disorders paroxysmal nocturnal hemoglobinuria (PNH) and myelodysplastic syndrome (MDS), their long-term incidence in patients treated with ALG, and a novel hypothesis on the pathophysiological relationship between SAA, PNH and MDS are presented. A case report written by one of Bruno Speck’s long-term patients, representing one of the numerous ‘off-protocol’ SAA patients, highlights the need for optimal individual treatment choices. This well-edited and timely publication is highly recommended to both clinical and experimental hematologists as a guideline for diagnosis and treatment and as a stimulator for further research aimed at improvement of treatment results - or prevention - of SAA.
