The authors of this text share their practical experience on implementing a multi-agency approach to the support of older people. The show how systems thinking can help with the complexity of developing a model of care that co-ordinates medical, social and community services. Christopher Foote was a consultant physician in General Medicine with a particular interest in the care of the elderly. He is currently an independent consultant in integrated care systems, having spent 15 years in their development. Christine Stanners trained and practiced as a hospital social worker. She became a manager in the voluntary sector and was project manager of the integrated care system for older people described in the book. She is currently involved in locality development.
