OverviewThis prospective, descriptive study was conducted on 50 patients followed for primary hypothyroidism under hormone replacement therapy at EPH-1 Arris (Batna, Algeria), between December 25, 2024 and February 4, 2025. The main objective was to analyze variations in ultrasensitive TSH in relation to associated comorbidities: type 2 diabetes, arterial hypertension and post-thyroidectomy status.The sample showed a strong female predominance, with a mean age of 47 ± 13.27 years. Statistical analysis using a one-way ANOVA test revealed that patients with type 2 diabetes had significantly higher TSH levels (7.85 mIU/L; p = 0.04), highlighting the complexity of thyroid balance in the presence of this comorbidity. In contrast, thyroidectomized patients had a near-normal TSH. The persistence of frugal hypothyroidism in 10% of cases confirms the difficulty of therapeutic optimization. These results underline the need for an individualized approach and rigorous biological monitoring of TSHus, particularly in patients with metabolic comorbidities.
