Hold the Blame Hunt to Learn - Facilitating Serious Event Review Teams offers fire department incident safety officers (ISO) and health and safety officers (HSO) information on how to conduct serious event reviews (often called investigations) after adverse events (accidents, injuries, close calls). The book describes how and why reviews should be performed using peer teams to analyze the internal decision making during the event. Hold the Blame Hunt to Learn describes how to avoid blaming individuals or groups and alternatively learn how and why the event took place once behavioral influences have been uncovered and identified. This book considers all the applicable investigative and review requirements outlined in NFPA 1021: Standard for Fire Officer Professional Qualifications, NFPA 1550: Standard on Fire Department Occupational Safety, Health, and Wellness Program, and NFPA 1521: Standard on Fire Department Safety Officer Professional Qualifications, insuring they are met or exceeded with this three-phase process.The process was developed by Deputy Chief Mark Keough, MPA, a 34-year veteran of the Mesa Fire and Medical Department, Arizona. A two-day course developed around this book is offered by the Fire Department Safey Officers Association in the U.S. and Canada.
