OverviewThis report discusses the June 6, 2000, fire that occurred on the Alaska Marine Highway System ferry Columbia, while it was underway in Chatham Strait, near Juneau, Alaska. From its investigation of this accident, the National Transportation Safety Board identified safety issues in the following areas: the adequacy of inspection and maintenance procedures for electrical systems; the adequacy of management safety oversight of maintenance procedures; and the adequacy of firefighting procedures. Based on its findings, the Safety Board made recommendations to the Alaska Marine Highway System.
