Many IT projects fail. Most of the failures are due to human error, misunderstanding, process problems and from time to time, technical gaffs. Root causes - systemic internalised practices that lead regularly to broken solutions - are analysed to find why what went wrong went wrong. Author Information

Karl A. Cox, PhD, is a Senior Lecturer at the University of Brighton, in the School of Computing, Engineering and Mathematics. He has taught business-IT alignment and project management to undergraduates and postgraduates for close to 20 years and has published over 80 articles and book chapters on the subject of IT and business requirements, alignment, processes and project management. Karl has also consulted to companies in the UK, Australia and Japan. He owned a consulting business in Australia specialising in business-IT alignment, and was a senior research scientist at NICTA, Ltd, in Australia. Whilst there, his focus was on the commercial development of methods to improve business-IT alignment, as well as conduct consulting and research engagements with companies across Australia and in Japan.
